When Sensitivity Becomes Paranoia: Cannabis, Alcohol and the Neurochemical Terrain
There is a particular kind of patient who arrives not because they are disconnected from themselves or their environment, but because they are in many ways too connected. They notice subtle shifts in tone, in timing, in relational presence. They pick up on inconsistencies, atmospheres, and undercurrents that others move past without registering. This sensitivity is often longstanding and forms part of how they orient to the world, and in the right conditions it supports depth of insight, emotional intelligence, and relational awareness. What brings them into difficulty is not the sensitivity itself, but the context within which that sensitivity is operating, particularly when the nervous system is no longer able to regulate, filter, or accurately contextualise what is being perceived.
At the centre of this shift is the brain’s process of assigning importance. Dopamine plays a central role in how the brain identifies what is meaningful, relevant, or requiring attention, and when this system becomes dysregulated the tagging of importance becomes amplified beyond proportion. Neutral or ambiguous stimuli begin to carry weight, not in a fleeting or dismissible way, but in a manner that feels compelling and immediate. A glance can feel intentional, a pause can feel loaded, and a coincidence can begin to take on personal significance. The mind then attempts to organise this heightened sense of meaning into something coherent, drawing connections and constructing explanations that account for the intensity of what is being experienced, and it is within this process that paranoia begins to form as an effort to stabilise and make sense of an internal state that has become difficult to contain.
This sits alongside a nervous system that is often already primed towards vigilance. The amygdala remains active, scanning for potential threat, while noradrenaline and cortisol maintain a level of physiological alertness that does not easily settle. Over time, the system becomes oriented towards anticipation rather than response, and this creates a background expectation that something may be wrong even in the absence of clear external danger. When heightened salience and heightened threat detection begin to interact, what feels important also begins to feel unsafe, and the body reinforces this through tension, activation, and a readiness to defend. The individual is therefore not only perceiving more, but perceiving through a system that is already biased towards identifying risk.
At the same time, the brain’s capacity to filter and prioritise information can become compromised. Glutamate-driven activity increases the volume of incoming signals, and the distinction between what is central and what is peripheral becomes less clear. Experiences that might ordinarily remain in the background move into the foreground, and the individual is left attempting to process a level of input that feels constant and demanding. This reduces the space available for reflection, making it more difficult to step back and consider alternative interpretations. Serotonin contributes to the emotional tone within which these interpretations are formed, and when its balance is affected, ambiguity becomes harder to tolerate and experiences begin to lean towards more fixed and often more concerning meanings. The prefrontal cortex, which would ordinarily provide a moderating influence and allow for perspective, becomes less able to regulate under these conditions, and thoughts begin to feel less like possibilities and more like conclusions.
Into this already sensitised and often overloaded system, the effects of cannabis can be particularly significant, especially in individuals whose baseline is already one of heightened awareness and vigilance. Cannabis interacts with the endocannabinoid system, which plays a role in regulating mood, perception, and stress response, and it also influences dopamine and glutamate activity in ways that can increase the assignment of salience and the intensity of internal experience. Thoughts can begin to feel more meaningful, more interconnected, and more urgent, while sensory perception can become more vivid and less easily filtered. In some individuals this is experienced as creativity or expanded awareness, yet in others, particularly where there is an underlying vulnerability in the nervous system, it can lead to a subtle but important shift in how reality is organised. The boundary between observation and interpretation becomes less stable, and the individual may find themselves moving more quickly from noticing something to drawing a conclusion about what it means.
Cannabis can also affect temporal processing and memory in ways that alter how experiences are held and revisited. A fleeting thought can linger and expand, and a moment of uncertainty can become something that loops and deepens. In a system that is already inclined towards pattern detection and meaning-making, this can lead to connections being formed that feel compelling but are not grounded in external reality. The experience is often not one of clear, linear thinking, but of an altered internal atmosphere in which things feel different, more charged, and less certain, and it is within this atmosphere that suspicion can begin to emerge and take hold.
Alcohol introduces a different but equally relevant dynamic. In the short term, its enhancement of GABA activity creates a sense of relaxation and reduced inhibition, which can feel relieving in a system that is otherwise tense or overactive. However, this effect is transient, and as alcohol is metabolised there is a rebound involving increased glutamate activity and activation of the stress response. Over time, even with patterns of use that are considered moderate, alcohol can begin to disrupt sleep architecture, affect serotonin balance, and reduce the overall resilience of the nervous system. Sleep becomes less restorative, emotional regulation becomes less stable, and the capacity to filter and contextualise experience can be subtly diminished.
In individuals who are already operating close to their threshold, this repeated pattern of temporary sedation followed by rebound activation can contribute to a system that feels increasingly unsettled and harder to regulate. The individual may notice an increase in internal noise, a reduction in clarity, and a greater tendency to interpret experiences through a lens of unease or concern. Because alcohol use is so socially normalised, these shifts are not always immediately recognised as being connected, yet within the broader neurochemical and nervous system context they can play a meaningful role in tipping the balance.
In clinical practice, what becomes important is not isolating one factor, but understanding how these elements interact within the individual’s overall terrain. Sensitivity, neurochemical regulation, nervous system state, relational history, and substance use all contribute to the way experience is shaped and interpreted. When this is approached with care and without judgement, patients are often able to see their experience in a new light, not as something inexplicable or overwhelming, but as something that follows a pattern that can be understood and gradually supported.
As the system begins to stabilise, whether through nervous system regulation, biochemical support, relational work, or changes in substance use, the intensity of meaning begins to soften and space returns between perception and interpretation. The individual regains the capacity to notice without immediately concluding, and to feel without being overtaken by what is felt. Sensitivity remains, but it is no longer carrying the weight of an overactivated system, and it can begin to function again as a form of depth, awareness, and connection rather than a pathway into fear.
